C.O.R.E – the Community Health Worker (CHW)

Be the first to experience C.O.R.E – the Community Health Worker (CHW) pilot training program that provides knowledge and skills development for individuals currently performing CHW duties, those interested in becoming CHWs, and other frontline workers that are working to improve the overall health of populations through effective connections and communications.


“Community Health Worker” is an umbrella term used to describe a variety of roles—both longstanding and emerging—in the health workforce.  Many different job titles fit the following American Public Health Association definition of CHW:

A community health worker is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.


In collaboration with the Community Health Worker Alliance of Iowa, the Iowa Chronic Care Consortium has created a training for Community Health Workers. C.O.R.E Training: Coaching, Outreach, Relationship and Engagement skills for community health workers, is a three-day onsite intensive training focusing on communication and engagement, as well as professional and technical skills.


C.O.R.E Objectives:

  • Define the scope of practice of a CHW
  • Explore professional, coaching, communication, and technical skills specific to their organizational roles
  • Develop participants’ ability to provide high quality, individualized support for improving the health of a population
