Nominate a Hero

We are now accepting nominations for the 2019 Iowa Public Health Heroes Award. We will accept nominations until Monday, Oct. 14, 2019. Recipients will be recognized at a College of Public Health Spotlight Series presentation on Dec. 4.

Each nomination should be accompanied by a nomination letter of no more than three pages, which you can upload via the form below. Multiple signatories may be incorporated into the letter. Evidence should include specific information regarding the accomplishments of the individual relative to the award criteria.
